Primary rhinoplasty
A primary rhinoplasty is for any patient who would be getting a nose job for the first time. Patients seeking a primary rhinoplasty are usually looking to correct a deviated septum, or to fix a previously damaged nose. In the first case, a deviated septum is when the bone and cartilage within the nose becomes more prone to being on one side of the nose or another rather than resting in the middle where it should be. This can result in difficulty breathing, loud breathing, and frequent nosebleeds. A primary nose job can take care of this by correcting the crooked portion within the nose by removing, or realigning it. In other cases of primary rhinoplasties, others may choose to simply improve the look of their nose for their personal satisfaction. They may have a slight hump they want to reduce the appearance of, or they may also want to even out the look of asymmetrical nostrils. In the second instance, someone who may have broken their nose before due to circumstances beyond their control may realize they want measures to be taken to fix it.
Revision rhinoplasty
A revision rhinoplasty is popular for patients who are either looking to fix ongoing issues regarding their first rhinoplasty, or for patients who would like to fix their noses based off of a prior injury. In the first case, someone may have thought they were making the right choice in choosing their plastic surgeon only to find out later that their doctor made a mistake or simply did not give them the results they were expecting. Incision options
Incision can vary based on the patient and what they are looking for in a nose procedure. We can starting discussing 3 outside incisions made during an open (external) rhinoplasty.
- Columellar
- Wedge
- Weir
A columellar incision cuts between the nose in the tissue connecting the nostrils while a wedge incision cuts in between the nose and cheek. Lastly, a weir incision cuts more closely towards the edge of the nostrils.
A closed (endonasal) rhinoplasty has quite a few options for types of incisions made within the nose.

- Transfixion
Starting with a hemitransfixion incision, this cut takes place in front of the septum where cartilage resides. Using an intercertilaginou incision, a cut is made between the upper lateral cartilage and the lower lateral cartilage. A Kilian incision cuts behind the caudal edge where cartilage resides. A marginal incision is made towards the end of the lower lateral cartilage. A rim incision is done at the end of the nostril, and a transfixion incision is made in both sides of the membranous septum by going through them. Incision choices made with a closed rhinoplasty are usually more popular because they conceal incision locations and scars.
